CLIENT HEALTH QUESTIONNAIRE PRIOR TO THE START OF MY SERVICE, I

CONFIRM THAT:


1. I have not been diagnosed with or cared for

someone diagnosed with COVID-19 in the past two

weeks.

2. I am not waiting COVID test results

3. I have not shown symptoms of COVID-19 or come

in close contact with anyone exhibiting these

symptoms in the past two weeks.

4. I have not traveled outside of my immediate daily

routine for the past two weeks.

5. I do not have a cough, fever, chills, shortness of

breath, or loss of taste or smell.

6. If I begin to show symptoms of COVID-19 within

the next two weeks, I will contact my service

provider

7. I will follow all posted salon rules to keep myself,





SALON CHANGES DURING COVID

 

We are so excited to see you! 


Please read these guidelines as your safety and comfort is of utmost importance to us and it is for that reason we have implemented safety and hygiene measures as per provincial regulations. Please take the time to read prior to your appointment.


-To reduce community spread, we ask that you wear a mask at all times while in the shop if you have an appointment. We will have some for sale including fashionable reusable ones at $23.99 and surgical disposable ones at $2.00, so do not worry!


-Please use our hand sanitizer upon entering.


-We can only accommodate a limited number of guests throughout the day in order to safely practice social distancing. We kindly request that only the booked client enter the shop. Drivers, dependents, friends, and family are to remain outside. Caregivers are welcome to assist but will need to remain outside until the end of the client’s appointment. Should you have a fever, shortness of breath, sore throat, feeling unwell the day of your appointment, we encourage you to contact us via phone or email to reschedule. -If you have been in close contact with someone who is sick or has confirmed COVID-19 in the past 14 days, or if you have returned from travel outside Canada in the past 14 days, you are not to book an appointment until after the 14 days and self-isolate during that time.


-Unfortunately complimentary coffee and tea will need to be on pause for now.


-Some of our prices have had to be adjusted

- 1 person at a time will be allowed to wait in our waiting area social distancing purposes.

-NEW- we can not accept any guests who have been travelling out of province, or southern Ontario. Please reach out to us if you have any questions. 


-If you have been exposed, waiting test results, or feeling g any symptoms, we can not provide any services.
